For my business i want to change my birth name, so i want to get a trade name, but i am suppose to write an Affidavit for it, but i don't know how to write a Name CHange Affidavit.
Dose any body know how to?? if you can provide me with a like sample of it or like instructions, i would truly appreciate it.

Actually you don't need to write an affidavit. You need to simply fill out the pre-made forms that are available based on your province. In BC you can go to any major police station and fill out the paperwork, bring current ID, get fingerprinted, pay for a criminal record check, etc and they send off the forms and within 2-8 weeks, you'll have a certificate with your new legal name.
